• Work alongside 11 primary care physicians and 20 Advanced Practice Clinicians in a collaborative care model using a Medical Home approach to care
• See 16-20 patients per day; full-time is 32 patient contact hours a week

Compensation and Benefits Details:
• Two-year base compensation of $300,000 + wRVU production bonus
• $200,000 recruitment incentive
