- Dual Trained Oral Surgeon
- DMD/DDS degree from an accredited dental school & MD/DO Degree with completion of head and neck fellowship
- Completion of an accredited Oral and Maxillofacial Surgery residency program
- Board certification or eligibility
- Valid state dental license and DEA license
