Position Details:
Hartford HealthCare is seeking a per diem Internist, Family Medicine or Geriatrician on a per diem basis to provide Skilled Nursing Facility (SNF) admissions and rounding coverage.
Role: Geriatrician
Location(s): SNFs in the Bloomfield area
Setting: 100% Skilled Nursing Facility (SNF) -patient admissions and rounding
Schedule: Per Diem, as needed (seeking 4-5 hours/day, 3-4 days a week)
Call: No call

- Supported by team of APP's that are in the facilities full time within a team of 18 Geriatric providers
