  • $10k sign-on bonus and negotiable relocation assistance

  • Must have Wisconsin license (or be willing to obtain)
  • Monday through Friday work schedule; 4-day work week option
  • Must maintain current BLS certification
  • Must be board certified; addiction fellowship
