Work Setting: Geriatric Medicine
RESPONSIBILITIES: Primary care. Medicare annual wellness visit and preventive services. Chronic disease management (e.g., arthritis, diabetes, heart disease)
Dates: ASAP through 2/27 & 3/13 - 3/21
HOURS: 8am-5pm
DAILY VOLUME: 8 patients/day
AGES SEEN: 63 and older
EMR: Centricity trained is highly preferred but not required
