Nuvance Health, is seeking Geriatricians/Medical Directors in Norwalk and Danbury, Connecticut, for our growing Geriatrics Program, including outpatient geriatric services, assessments, geriatric wellness, care in SNF and assisted living settings, as well as supervision of advanced practitioners. This role will encompass work with our contracted SNF and LTC facilities in Western Connecticut in the Norwalk and Danbury areas.

The ideal candidate will be board certified/eligible in Internal or Family Medicine. Geriatric fellowship training and /or experience preferred as well as experience working in SNF settings. 

This is an employed position with Nuvance Health Medical Practice. Competitive total compensation package including base salary, incentive pay, paid time off, CME time off and allowance, medical, dental, vision, life, disability, 401k match and contribution. Hiring bonus and relocation stipend may also be available.

Job Requirements:

• MD or DO with CT State License, Federal DEA and State Controlled Substance Registration
• Successful completion of Internal Medicine Residency; Geriatric Fellowship training preferred
• Previous work experience in SNF settings preferred
• Must be Board Certified or be Board Eligible
• Off-hour coverage – Shared among multiple providers, phone-calls only with nurse triage
• Willingness to work Monday through Fridays 8:00 AM – 5 PM.
