Tufts Medical Center Community Care is seeking to hire an Internal Medicine / Geriatrics / SNF Physician to join our expanding team of collaborative physicians.

We are committed to compassionate care in our communities and have a well-established care team to serve individuals in skilled nursing facilities with a focus on quality of life and compassionate, collaborated care.

Providers on this team work closely with hospitalists, emergency medicine, hospice and palliative medicine, primary care, and specialists to ensure continuity of care.

Tufts Medical Center Community Care is affiliated with MelroseWakefield Hospital and Lawrence Memorial Hospital of Medford, with access to specialists at Tufts Medical Center and Tufts Children's Hospital for highly specialized care.

How you'll transform patient care:

  • You'll be provided with an existing base of facilities and patients
  • You'll have support in expanding the facilities at which we provide care
  • You'll hone your leadership skills through opportunities in driving critical change & process improvements while collaborating with Nurse Practitioners and clinical support staff
  • You'll grow as a physician with the opportunity for Medical Directorship
  • You'll be a strong voice in developing the program for frictionless care between facilities and the hospital
  • You'll have flexibility to get creative with your schedule
  • You'll use a fully integrated EMR with Dragon dictation available
  • You'll focus on quality, panel size and coordination of care, not just productivity

Who you are:

  • BE/BC in Internal Medicine or Family Medicine
  • Believe patient-centered care and evidence-based medicine are the secret sauce to building trusting relationships with families in our communities
  • Committed to a team-based care approach with an entrepreneurial mindset
  • Champion of learning new technology and navigating EMR systems
  • Experienced in primary care, hospital medicine, or geriatrics is a plus
  • Current medical license in the Commonwealth of Massachusetts is a plus
  • Completed fellowship in Geriatrics is a plus
