Position Summary:
Yale New Haven Hospital / Northeast Medical Group seeks a qualified applicant to join our practice in a unique, dual hospitalist-palliative medicine position. Join a growing palliative care program at The Saint Raphael Campus, in collaboration with the Yale New Haven Hospitalist Program and Yale Palliative Care Department. An ideal candidate will demonstrate commitment and interest in providing high quality palliative care to hospitalized patients and have strong skills in hospital-based general medicine. Multiple leadership, educational, and academic opportunities exist within the medical center, including opportunity for Clinical Faculty Appointment at Yale School of Medicine and opportunity for program building in palliative care. Responsibilities include: attending on the hospital-based palliative care consult service, attending on the hospitalist service, and serving as a bridge to the hospitalist program to strengthen the primary palliative care skills of the group.
The staff hospitalist must be able to competently care for the hospitalized patient throughout the hospital stay. This includes emergency evaluation and triage in addition to admission to the hospital and formation of a differential diagnosis. It is essential to formulate a comprehensive care plan in accordance with the most up to date, evidence based protocols. The hospitalist physician should participate in education of the house staff physicians, students, nurses and all members of the patient care team. The hospitalist should keep high quality care and patient safety in mind while transitioning the patient back to the outpatient setting upon discharge. Participation on Yale New Haven Hospital committee's service is encouraged in an effort to demonstrate leadership.
Qualifications/Education:
Medical Degree (MD, DO) and Internal Medicine Residency completion.
Board certification or board eligibility in palliative medicine.
