Job Details
PRIMARY RESPONSIBILITIES
Sets post-acute and cross continuum quality/patient safety/value metrics in collaboration with System clinical leadership, ensuring alignment with System quality/patient safety/value metrics. Ensures alignment of these goals with the Key Performance Indicators (KPIs) of SSM Health and cascades goals throughout the system using SSM Health Continuous Improvement Operating Model.
Works collaboratively with Administrative and other Medical Director Leaders to set the strategic vision, goals, and objectives for post-acute care and ensures alignment with SSM Health strategy. Participates with Clinical Program Medical Director physician leaders and System Clinical Operations Leadership in setting the Clinical Program goals and objectives.
Identifies opportunities for operational efficiency and coordinated care across specialties and geography in association with other Regional Clinical Program and Administrative Leaders with appropriate implementation of processes to improve patient outcomes and experience.
Helps develop and implement a strategy for acute care transitions to decrease readmissions and improve patient experience/outcomes,
Ensures support is provided to Specific Clinical Programs, as indicated, to ensure implementation of appropriate care standards across the care continuum with safe and efficient care transitions
Participates in the assessment and implementation of external business opportunities and partnerships based on the priorities of SSM System and post-acute care strategies. Examples might include management partnerships, senior living/services expansion (assisted living, memory care), Hospital-at-Home, etc.
Develops integration of other system initiatives, e.g. antimicrobial stewardship that improve the outcome for patients who utilize post-acute care services.
Works with appropriate leaders including Mission to develop a comprehensive palliative medicine program which addresses acute care, supportive care clinics (e.g. oncology), home palliative care, and hospice.
Provides leadership and oversite for all physician and APP support for post-acute services, including home health, hospice, palliative care, and skilled nursing facilities.
Works with other leaders to develop plans for expanded senior services, including strategy and implementation of priorities for providing high quality care to seniors, consistent with the SSM Senior Services strategic plans.
Provides expertise, knowledge, and assistance in matters related to clinical staff safety, quality, risk management, and utilization review activities.
Responsible for driving, supporting and modeling a service-oriented culture focused on employee engagement, quality, patient safety, and service.
Performs other duties as assigned.
Education
Medical Doctorate or Doctor of Osteopathy Degree required with Board Certification. Must be eligible for or hold a current and unrestricted State license to practice medicine.
Experience
Five years' experience in clinical practice with three years of progressive leadership responsibilities or positions. Prior physician leadership experience would include, for example, a Medical Director or CMO for a large multi-specialty group practice, physician practice management organization, hospital/hospital system, or health plan.
Proven track record leading physician groups, including group facilitation, leadership development, team-building, performance management, and driving cultural change. Experience working with physicians to strategize and develop measurable improvements in the quality of clinical care, affordability, and patient satisfaction. Proven experience implementing a continuous improvement culture with establishment, achieving, and sustaining appropriate process and outcome measures for key initiatives.
Experience navigating a highly-matrixed organizational structure with skill and efficiency
