Geriatric Medicine Physician (Providence ElderPlace - Oregon)

Seeking full-time board-certified/board-eligible Family Medicine or Internal Medicine trained physician, or completion of Geriatric Medicine fellowship. Successful candidates will have two years of recent clinical experience working with geriatric population. At Providence ElderPlace Gresham, you'll join an established interdisciplinary team providing older adults with a single, coordinated program of comprehensive care and social services. • Full-time 1.0 FTE, 5 days per week
• Monday to Friday, daytime hours
• Call schedule 1-2 week nights per month; 1-2 weekends per quarter
• Outpatient primary care setting, some home visits required
• Interdisciplinary care team including behavioral health, clinical pharmacist, nursing staff, social workers and rehab
• Two years of recent geriatric practice experience required
• Ideal candidates will have training and/or experience in providing Palliative Care
