Agnesian Healthcare, a member of SSM Health, is actively seeking an experienced physician leader that is clinically outstanding, possesses a high level of empathy and caring, and has a passion for educating providers and staff on Hospice Medicine. This Hospice Medical Director will be focused mainly on administrative tasks associated with supported our Hospice patients and staff which includes both hospice locations and inpatient support. A high degree of success contingent upon relationship building capabilities with our primary and specialty care providers in outpatient, inpatient, skilled nursing, and home settings ensuring continuity of care. This role will be based in Fond du lac, WI and will have a small clinical component.

Position Specifics:

1.0 FTE
Patient and staff support
Collaborate with palliative and primary care providers across the greater Fond du lac region
Provide program oversight for organization
Represent SSM WI Region in organization-wide Hospice Medicine Initiatives

Requirements:

M.D. or D.O.
Completion of AOA or ACGME specialty of subspecialty residency program
Board Certified in Hospice/Palliative Medicine or completion of Hospice/Palliative fellowship
Active WI Medical License
Active DEA Certificate
Experience with both in home and outpatient hospice Medicine
Demonstrated success in developing and overseeing a Hospice division/department
