Capital Health is seeking a full-time, board-certified Lead Geriatrician for its Nursing Home practices to provide post-acute and rehab care throughout our community nursing homes. 

Education, Training & Experience:
  • Board-certified in Geriatrics or board-certified in Internal Medicine or Family Medicine with geriatric experience. 
  • Palliative care experience a plus, but not required.
  • Eligible for licensure in the state of New Jersey.
  • Must possess strong interpersonal skills and ability to maintain a high level of satisfaction with referring physicians/institutions. 
  • Five years of experience in a community clinic and/or family practice clinic setting.
  • Three years of supervisory experience.

Position Highlights:
  • Joining 6 other full-time physicians and 1 nurse practitioner.
  • Cultivate relationships and facilitate partnerships between Capital Health Medical Group and post-acute care/long-term care facilities, assisted living facilities and acute rehabilitation facilities.
  • Conduct periodic performance evaluations on APPs and physicians in the post-acute care practice.
  • Arrange for and participate in after-hours coverage of patients in nursing homes, sub-acute rehab facilities, long-term care facilities and anywhere else that patients of the practice are located.
  • Coordinate and improve communication amongst hospital, post-acute facility staff, hospitalists, primary care physicians, Emergency Department and Care Management to enhance patient care quality across the continuum.
  • Assist with the establishment and enforcement of clinical and quality control programs, policies and procedures to ensure diagnostic and therapeutic reliability and safety of patients and personnel.
  • Active participation in quality improvement initiatives. This can occur by involvement in hospital-based committees, project leadership or pilot programs that are intended to advance the care of patients.
  • Collaborate with physicians and APPs on all admissions to partner facilities.
  • Develop and/or approve such clinical and operational protocols, procedures, rules and regulations as may be necessary.
  • Conduct regularly scheduled meetings with providers on system issues regarding patient flow, quality, performance and improvement, and promotion of professional development.
  • Utilize data from the post-acute care program to enhance quality of the operation and strive for a high-reliability program and culture.
  • Create/build a process and infrastructure to facilitate safe transitions of care between inpatient post-acute care, outpatient and home.
  • Provide input and advice regarding patient flow and accurate transmission of appropriate patient information to improve safe handoffs and transitions of care for patients both coming from and returning to the hospital and post-acute care facilities.
  • Enhance palliative care, including POLST review, as appropriate for patients in post-acute settings.
  • Reduce unnecessary hospital service from post-acute care and long-term care facilities when appropriate care can be provided in the post-acute care/long-term care facilities.
  • Enhance level of care provided in the post-acute care/long-term care facilities.
  • Create clinical pathways specific to medical conditions/disease states.
