Responsibilities
The Tele-Nephrology Director will work closely with the VISN 21 Tele-Health Coordinator to ensure the quality and timeliness of services provided and that those services are provided in a Veteran centric model. The Tele-Nephrology Director will collaborate closely with the VISN 21 Specialty Care Integrated Clinical Community (ICC) Lead. This position will report to the VISN 21 Chief Medical Officer (CMO) and will support clinical and administrative tele-nephrology across seven VISN 21 facilities and affiliated outpatient clinics. Duties include, but not limited to:

* Participation in staff meetings, hospital committees, and peer review process, clinical directorships as may be assigned by the VISN 21 CMO.
* Demonstrate effective communication both down to staff and up to leadership, to include heads up (for adverse patient bad outcomes) and issue briefs.
* Timely completion of documentation and upkeep of credentials required for privileging and re-privileging.
* Participate in Clinical Pertinence Reviews (routine medical record reviews).
* Conducts open encounter & open consult management, CPRS documentation rules and requirements, adherence to clinic procedures for patient scheduling & access, ensuring staff productivity, oversight of relevant performance measures, participation with electronic communication and health management services and processes, and participation in the development of the new focus on the Medical Home and Patient Centric Care, Utilization, Quality, and Patient Safety oversight, process improvement (system redesign).
* Provide patient care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness, treatment of disease, and care at the end of life.
* Demonstrate knowledge of established and evolving biomedical, clinical and social sciences, and the application of their knowledge to patient care and the education of others.
* Use scientific evidence and methods to investigate, evaluate, and improve patient care.
* Demonstrate interpersonal and communication skills that enable them to establish and maintain professional relationships with patients, families, and other members of the health care team.

* Conduct physical examinations and treatment, coordination of medical care with primary care providers, other subspecialists, social workers, data managers, pharmacists, dietitians, nursing, and other personnel as a leader of a multidisciplinary team.
* Utilize virtual care to provide care to remote clinic locations.
* Ensure that high quality patient care is delivered in a resourceful and efficient manner.
* Evaluate, diagnose, and manage patients to include but not limited to those with acute and chronic renal disease, hemodialysis and renal transplant patients.
* Provide patient care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness, and treatment of disease.
* Demonstrate interpersonal and communication skills that enable them to establish and maintain professional relationships with patients, families, and other members of the health care team.
* Provide resident and/or trainee supervision, as necessary and/or appropriate.
* Work collaboratively with other specialists, and Primary Care, to ensure patients receive continuous quality care and to provide nephrology training as requested.
* Function as the expert for patient care related to acute, chronic, and end-stage kidney disease, fluid, electrolyte and acid-base disorders.
* Generate appropriate consultation and follow-up note documentation, order prescription medications, provide diet recommendation and instruction, and other physician-driven or exclusive decisions.
* Other duties as assigned by VISN 21 CMO.
Work Schedule:Full Time, 40 hours per week, Monday-Friday, 8:00 am - 4:30 pm; specific tour of duty may be negotiated upon entry.

Basic Requirements:
* United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
* Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
* Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
* Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
* Proficiency in spoken and written English.

Preferred Experience:The incumbent is a physician and has completed a residency training program in Internal Medicine. A minimum of 5 years of successful professional practice and 3 years in administrative positions with progressively more complex leadership assignments is recommended. Physical Requirements:
