Sea View Hospital and Rehabilitation Center and Home is a 304-bed long term care facility located in Staten Island's lush Greenbelt. Serving the metropolitan area, it is easily accessible from all points by car and bus. Like many HHC facilities, Sea View has a long history. It opened its doors in 1913 as a sanitarium treating individuals suffering from the most feared disease of the time, tuberculosis. Today, the hospital is well-known for its long-term care brain injury unit, the first in New York State. Patients undergo intensive rehabilitation with the goals of restoring motor and cognitive functions and maximizing quality of life.
Responsibilities:
- Provide medical management for geriatric and non-geriatric nursing facility residents including primary care, management of acute illness, and transition of care.
- Coordinate behavioral health services for individuals with maladaptive behaviors.
- Carry a patient case load.
- Participate in cross coverage for absences of other team members.
- Participate in weekend and holiday coverage and off-site telephone coverage on a rotational basis.
- Perform medical assessment for new admissions and readmissions and write orders for required medications, treatments, diagnostic studies and other necessary care and treatment.
- Perform required physician visits based on resident need, facility policies and regulatory requirements.
- Write timely orders and progress notes.
- Initiate referrals for consultant and specialty services as required and write orders for services needed.
- Write orders for rehabilitation services.
- Participate in advance directives planning with the resident and representatives and write related orders.
- Participates in discharge planning/transition planning process and provides medical documentation as required.
- Facilitate the emergency discharge of residents to acute hospitals as well as planned discharges for medical treatment
- Participate in interdisciplinary rounding and meetings.
- Actively involved in the unit and facility performance improvement process.
- Provide medical leadership for team-coordinated Quality Assurance Performance Improvement initiatives
- Other assignments as designated by the Chief Medical Officer.
Minimum Qualifications:

Department Preferences:
- Long term Care experience. Geriatric Board certified.
- Experience with Post-Acute EMR
- Familiarity of CMS and NYS DOH requirements for the provision of physician services in adherence to medical management of nursing facility residents.
