REQUIREMENTS

Candidate to be a medical doctor duly licensed by the State of Florida Department of Health.

Candidate to be duly authorized by the State of Florida Department of Health to engage in every aspect of the practice of medicine and to focus his/her practice in the area of Endocrinology, Diabetes, and Metabolism. Candidate to actively practice adult Endocrinology, Diabetes and Metabolism in an outpatient practice located at 1667 North Clyde Morris Boulevard, Suite 2, Daytona Beach, Florida, 32117, solely on behalf of the EMPLOYER and for a minimum of forty (40) hours per week, Monday to Thursday during normal clinic hours.

The candidate will provide, at a minimum, the following services within the scope of medical training and subject to the ethical standards imposed or prescribed by the Joint Commission on Accreditation of Health Care Organization the American Medical Association, Medical Professional Associations of candidate s scope of training, the Medical Society of the State of Florida, and the State of Florida Department of Health, Board of Medical Examiners:

Endocrinology, diabetes and metabolism outpatient diagnosis, management, care and non-surgical treatment of any endocrine system of all adult patients of the PRACTICE requiring attention in accordance with the prevailing standard of care in the community.

Procedures in accordance with the candidate s clinical skills and capabilities of the PRACTICE.
Keeping and maintaining or causing to be kept and maintained appropriate records, including without limitation patient charts, electronic medical records of the PRACTICE on a timely basis. Also, being in accordance with professional, governmental, third-party payer, and record keeping and reporting requirements.

Collaborate, participate and organize prevention and research programs in diabetes, endocrinology, nutrition, metabolic syndrome and atherosclerosis, clinical research and academic teaching activities of the PRACTICE.
