Responsibilities
This position is designed for approximately 50% time in Community Living Center (CLC) nursing facility care and approximately 50% time in geriatric ambulatory care, in the setting of a Geriatrics-PACT (primary care team, patient-aligned team). There are currently three Community Living Centers serving veterans in Topeka, serving a mix of patient types. A newly renovated unit is coming on line which will provide skilled nursing facility/post-acute care and hospice care, which will initially occupy a majority of this position's time. The GeriPACT is not yet launched. The distribution of effort between various geriatrics care settings in the Geriatrics and Extended Care (GEC) service line will flex as needed during the build-phases of these programs. Working collaboratively with other Physicians, Nurse Practitioners and/or Physician Assistants is required. The incumbent must be able to work collaboratively with multiple health care disciplines, as well as with facility leadership and non-clinical areas involved in providing infrastructure for safe patient care.

Clinical Duties:include, but are not limited to: Provide high quality patient care in variety of settings as above; Serve as a key clinical resource for matters involving geriatric services; Provide comprehensive care for our ambulatory patients including diagnosis and ongoing treatment of assigned patients; Provide effective medical treatment, consultation, and diagnosis upon request from other Services/Departments; Order lab tests, x-rays, EKG's and other diagnostic procedures; Initiate or expedite requests for consults, special tests, or studies; Coordinate with

Community Care

[HCA1]for services provided outside the VA health care system; Assess Veterans by utilizing advanced clinical judgment and physical assessment skills; Identify acute and/or chronic health problems; Assess and manage geriatric syndromes such as falls, gait and balance disorders, failure to thrive, incontinence, memory loss, including dementia, polypharmacy, depression, osteoporosis and sleep disturbance. Coordinate with pharmacy for medication reconciliation, and polypharmacy redirection; coordinate with social work and psychology and nursing and other health disciplines to meet patient needs. Complete all documentation and encounter information in a timely manner.

In addition to face to face patient care, the incumbent will also participate in all required elements of indirect patient care such as interdisciplinary team meetings, care planning meetings, open encounter & open consult management, CPRS documentation rules and requirements, adherence to clinic procedures for patient scheduling & access, ensuring staff productivity, oversight of relevant performance measures, and participation in the system redesign processes. Incumbent will arrange coverage as needed and communicate it to team members and will participate in coverage of other Geriatrics and Extended Care (GEC) providers as needed, including managing their CPRS alerts and other related cross coverage tasks. Will participate in education of geriatric fellows and other learners if assigned to relevant rotations in their work area, and fulfill related educational administrative tasks as required.

Other Duties:The incumbent also participates in leadership responsibilities as assigned, including but not limited to participation in staff meetings, committees, and peer review processes. Will report clinical events to leadership, including those requiring issue briefs. Will maintain all VA training requirements and those of all members of the Medical Staff. Will maintain all credentials required for licensure, privileging and re-privileging, ability to prescribe medications, and related requirements for clinical care without interruption. Will participate in initial and ongoing professional practice evaluation processes and in chart review processes. Will adhere to employee health requirements. Will demonstrate adherence to tour of duties, timely requests for leave and for clinic cancellations, and other administrative requirements for VA employees.

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 03/16/2021.
Basic Requirements:
* United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
* Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
* Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
* Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
* Proficiency in spoken and written English.

Preferred Experience:Board Certification preferred.

Physical Requirements:This position requires light to moderate lifting (15-45 lbs); light carrying (15 lbs and under); reaching above shoulder; walking; bending; stopping, and some lifting in the direct care of patients; ability for rapid mental and muscular coordination simultaneously; hearing (aid permitted).Education
