We are looking for a physician with a passion for improving the quality of life for our most vulnerable population and their families, to work in the Post-Acute and Long-Term Care arena. The Central Valley is seeking either a Geriatrician or IM or FM-trained physician that will provide care for patients undergoing skilled rehab as well as those in long-term care. A Geriatric-trained physician will also cover the dementia clinic 2 half days/week and join the team developing additional outpatient geriatric services.
Physicians will join a stable group of Geriatric-trained MD s to cover a census of approximately 20-skilled patients and follow about 30 long-term care patients based in facilities around the Stockton, Modesto or Manteca areas. They will join a skilled team including 1 NP providing labs/call & skilled visit support, 1 LVN, 1.5 MAs, and 1 administrative professional. Clinicians will be provided a Kaiser laptop, iPad and iPhone.
