Reporting to the Medical Director, you will help deliver on our promise to the people of Brooklyn by providing exceptional patient care and wise counsel to elderly/senior patients with chronic conditions or multiple health problems. In this dynamic and richly gratifying role, you will also: 

  • Make determinations on patient’s ability to live alone safely (or be referred for alternate living)
  • Provide diagnoses, treatment and patient/family education that enable a better quality of life
  • Discuss end-of-life care with patients and families
  • Use evidence-based guidelines to plan and deliver patient care and treatment
  • Order, interpret and discuss the results of diagnostic tests
  • Collect, record and maintain patient data
  • Refer patients to specialists for consultative services
  • Demonstrate great respect and concern for patients, family members, guardian, health aides, etc.
  • Prescribe medication, vaccinations, and specialized or preventative care
  • Monitor compliance with Infection Control and Safety and HIPPA standards
  • Build collaboration and successful relationships with medical, support and administrative staff
  • Participate in in-service education and attend continuing medical (CME) programs
  • Assist with teaching, evaluating and mentoring residents in the Family Medicine Residency Program
  • Supervise residents and medical students
  • Accurately document all supervision and oversight activities in the medical record
  • Attend meetings and committees as requested by the Medical Director and/or Department Chair
  • Continuously seeks new ways to improve processes and outcomes 

The ideal candidate will have:

  • MD or DO degree with at least two years of post-residency medical/clinical experience
  • Current NYS Physician License and DoH Certificate of Qualification
  • BC/BE in Geriatric Medicine; ACLS and BCLS training
  • Participated in direct education of residents during didactic sessions and by providing direct patient care
  • Familiarity with the competency-based curriculum for residency training
  • Credentials or eligible to be credentialed in the Medicaid Program in New York
  • Exceptional interpersonal, oral and written communication skills
  • Demonstrated experience promoting a diverse workforce and positive interactions with students, residents, interdisciplinary team members and faculty
  • Most significantly, a mindset oriented to returning patients to high functionality and quality of life
  • The capacity to maintain patient’s confidentiality always
