North Memorial is looking for a BC Neuro or PM&R locum for December 5, 2019 to February 14, 2020 as well as for ongoing staff vacation coverage which would total approximately 8 weeks a year of coverage.

Additional Details:

• Prefer PM&R but will consider Neuro provider who has rehab experience.

Shift(s) Description:

• Dec 5-13, 2019

• Jan 27- Feb 14, 2020

• 3 month Credentialing

• Average # of office/clinic patients per day: 4-7 (scheduled)

• Average # of hospital based patients per day: 1-3 (admissions) 11 (rehab)

• Procedures: Botox and Intrathecal Pump Management

SUBMISSION REQUIREMENTS:

• Board Certified or Board Eligible Neurologist (will consider PM&R physician)

• STATE LICENSE REQUIREMENTS: Minnesota

Program Details:

• Comprehensive and intensive therapy 3 hours per day, 6 days per week

• In-depth education for customers, families and caregivers of recovery process

• Individualized care plans created by our on-site neuro-rehabilitation physician
