OPPORTUNITY HIGHLIGHTS 
 Eddy SeniorCare, a member of St. Peter's Health Partners, is an established and respected PACE program (Program of All Inclusive Care for the Elderly). Eddy SeniorCare is seeking experienced Family Medicine, Internal Medicine, Geriatric Medicine or Hospice and Palliative Care physicians to fill Medical Director roles for locations in the Greater Albany area, New York.  

The PACE program is seeking two Medical Directors for two separate locations in the Greater Albany area (better known as the Capital District). One physician will be based at the new day center that will be opening in late 2019 in Latham and one physician will be based the brand new offices and day center in Rotterdam. The selected physicians' will carry the title of medical director and have some quality assurance, infection control and administrative responsibilities.   

Eddy Senior Care is a unique model of care—the only one of its kind between New York City and Canada—and expanding into two new counties. Soon to be serving Albany, Rensselaer and Schenectady Counties. 

The selected physician will understand and appreciate the positive impact comprehensive medical care outside of the hospital setting can have on seniors from lower socio-economic backgrounds. Will prefer to work in an environment where the productivity expectations of traditional outpatient primary care do not exist, and doctors can spend more time with patients, based on their needs. Would like to be part of a program that represents the future of health care in the United States: having the latitude to creatively craft broad and thorough care plans for each participant because we are both the payer and provider, accepting lump sum payments from Medicare and Medicaid and going at risk for quality and value. 

The selected physician must be Board Certified in Internal Medicine or Family Medicine with a minimum of one to three years of experience with a Geriatric population.  A strong preference will be shown to candidates who possess fellowship training in Geriatrics or Hospice and Palliative Care. Candidates should possess excellent clinical skills, communication skills, and a strong commitment to providing compassionate care, and the ability to work in a team environment.
