About the organization

Santa Clara Valley Health and Hospital System (SCVHHS) is the second-largest County-owned health and hospital system in California and is committed to improving the health of the 1.8 million people of Santa Clara County.  As an integrated health care system, SCVHHS includes a 574-bed central hospital (SCVMC), a large primary care network comprised of nine health centers throughout the County (including our newest center in downtown San Jose, which opened in 2016), a broad-range of specialty services in our Valley Specialty Center, a large behavioral health department, public health, EMS, and Valley Health Plan.

About the Division

Our Geriatric practice is located in the beautiful Downtown San Jose in the heart of Silicon Valley.

Our division provides comprehensive primary care to the most frail elderly and consultative services to seniors in the County of Santa Clara. We are very passionate about supporting our patients in the continuum of care as well as strive for safe transitions of care between the different settings.

Our interdisciplinary team is very collegial and strong mentoring support is available from committed physicians to ensure excellent career development.

Physicians joining our group are pleased to find supportive specialists in the Departments of Medicine, Surgery, GYN as well as an efficient, integrated electronic health record (EPIC).

About the Position

The Division of Geriatrics Primary Care is seeking a fellowship trained Geriatrician to join our established Comprehensive Geriatric Care Clinic.  Applicants should have a current CA medical license, DEA and eligibility to participate in Medi-Cal/Medicare.

The successful candidate will work closely with our robust interdisciplinary team to provide longitudinal primary care to our diverse, frail older adults, as well as assist our primary care, specialist colleagues, and community partners with geriatric consultative services.  There is also opportunity for QI projects for the division as well as teaching IM residents.

About the Department of Family Medicine

As the newest department at SCVMC, the Department of Family Medicine with 20 family medicine physicians provides primary care services at seven health centers, including mature family medicine practice models in Milpitas and Gilroy.  Family physicians who join our department are pleased to find a very collegial work environment, with robust specialty and ancillary support.
