Must possess a Basic Life Support card from the American Heart Association
Must possess a board certification in Family Medicine as determined by the American Board of Family Medicine or from the American Osteopathic Association.
Must possess current, valid, unrestricted license in one of the fifty states, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands
Must be a graduate from a medical school approved by the Liaison Committee on Medical Education of the American Medical Association or the American Osteopathic Association or certification by the Educational Council for Foreign Medical Graduates (ECFMG).
Must have successful completion of a residency program which has been approved by the Accreditation Council for Graduate Medical Education or the Committee on Postdoctoral Training of the American Osteopathic Association or those Canadian training programs approved by the Royal College of Physicians and Surgeons of Canada or other appropriate Canadian medical authority.
Must possess a minimum of 3 years’ experience as a Primary Care Physician within the last 5 years.
