LIFE at St. Joseph of the Pines (SJP) has an excellent opportunity for a Board Certified or Board Eligible Internal Medicine or Family Medicine physician, preferably fellowship trained in Geriatric Medicine to join the Program of All-Inclusive Care for the Elderly (PACE) facility located in Fayetteville, North Carolina.

The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. LIFE serves individuals who are age 55 or older, certified by their state to need nursing home care, able to live safely in the community at the time of enrollment and live in a LIFE service area. While all LIFE participants must be certified to need nursing home care to enroll in LIFE, only about 7 percent of LIFE participants nationally reside in a nursing home. If a LIFE enrollee needs nursing home care, the LIFE program pays for it and continues to coordinate the enrollee's care. The program encourages fellowship and independence while serving seniors and their families with dignity and respect. Delivering all needed medical and supportive services, LIFE is able to provide the entire continuum of care and services to seniors with chronic needs while maintaining their independence in their home for as long as possible.

This is a full time employment opportunity with a four day work week. The physician will report to the Medical Director, while administrative supervision of this physician will be that of the Executive Director. Under the supervision of the Medical Director in coordination of the Executive Director, the selected physician will provide primary medical care to LIFE at St. Joseph of the Pines participants, including assessment, development of a plan of care in collaboration with the interdisciplinary team, provision of direct participant medical care, and evaluation of effectiveness of the plan of care. They will be expected to function in a collegial relationship with the other health care professionals, making independent decisions regarding medical needs and health regimens while carrying out medical procedures as indicated.

Responsibilities of this opportunity include the following:

  • Perform physical examinations, order and evaluate appropriate laboratory and diagnostic tests, and record results in a systematic manner.
  • Provide initial and ongoing participant assessments, care plan development, and implementation of care plan.
  • Provide preventative health care and health promotion for LIFE at St. Joseph of the Pines participants.
  • Educate participants and families regarding health maintenance and their chronic medical conditions, in cooperation with nursing staff.
  • Serve as a member of the interdisciplinary team and participate in the development of the comprehensive plan of care and ongoing monitoring of the participant’s health status. Maintain an active role in the interdisciplinary team process.

The selected candidate must be Board Certified or Board Eligible in Internal Medicine or Family Medicine with a preference shown to candidates who possess a certificate added qualifications in Geriatric Medicine. Candidates must possess or be eligible to obtain a medical license in the state of North Carolina, an active DEA registration and staff privileges at the LIFE at St. Joseph of the Pines contracted hospitals. The successful candidate will have an interest in frail older adults and knowledge of their physical, mental, and social needs as well as a minimum of one year of documented experience working with a frail or elderly population. They must have effective skills in physical assessment and chronic disease management for frail older adults as well as the desire to work in a creative alternate health care setting.

ABOUT THE FACILITY

Living Independently for Elders (LIFE) is a Program of All-inclusive Care for the Elderly (PACE) at SJP, which assists older adults in living safely at home.  Eligible participants are 55-years old or older, live in the designated service area, meet the state requirements for nursing home care and are able to live safely in the community.

LIFE is designed around the needs of the individual and provides comprehensive medical, health and social services (including adult day health programs, rehabilitation therapy, nutritious meals and social engagement) to participants at the center as well as in their homes.  The growing program currently supports 220+ participants and is expecting expansion efforts in the near future.
