BC/BE Geriatrician

The Baptist Health AgeWell Institute, located in downtown Jacksonville, is seeking an additional geriatrician to join a rapidly growing, innovative senior health practice. The AgeWell Institute was formed in October 2012 and is the only comprehensive, interdisciplinary geriatric program in Northeast Florida.

A fellowship-trained, board certified or board eligible Geriatrician is preferred to help shape our comprehensive, innovative programs and Level 3 Certified Patient Centered Medical Home. We will also consider family medicine or internal medicine physicians with a strong background in serving the senior population.

Join a group of two physicians and help support a developing house call program with a nurse practitioner driven model.  Limited house calls will be required and shared within the group. This is a full time, outpatient position with minimal call responsibilities shared with our primary care provider group. No weekends. 

Qualifications

• MD or DO
• Board Certified or Eligible
• Active/Unrestricted medical license
• Geriatric Fellowship Preferred
• Internal Medicine or Family Medicine with experience serving a senior population is acceptable
