I'm Dr. Reginald Ross, an Internal Medicine Specialist at CenterWell Senior Primary Care in Louisiana. Throughout my career, I've explored various care models, but finding a place that genuinely supports both patient outcomes and physician well-being was important to me. At CenterWell, I’ve experienced the difference a true patient-centered, value-based care model can make—not just for my patients, but for me as a provider.

Throughout my career in health care, I've had the opportunity to experience both the traditional fee-for-service and the innovative value-based care models. My career journey thus far has shown me the stark contrasts between these approaches and highlighted the significant benefits of value-based care, not just for patients but for providers like myself.

Earlier in my career, I practiced under the fee-for-service model. In this environment, I routinely saw over 25 patients a day, which led to long working hours and a constant, never-ending struggle to manage and avoid burnout. The high patient volume, alongside the necessity to juggle numerous administrative tasks, made it challenging to develop meaningful relationships with my patients. While some physicians thrive in this environment, I found it less conducive to providing the kind of comprehensive care I aspire to deliver.

Attracted by the opportunity to provide better patient outcomes and more personalized care, I explored value-based care delivery models. These models focus on a patient’s holistic health and well-being, emphasizing preventive measures and comprehensive care plans tailored to patients’ specific needs and lifestyles. However, in my search for the perfect fit for my patients and me, it was clear that even within some value-based systems, there were still high patient daily visits and consistent pressure on physicians to deliver exceptional outcomes without adequate resources to deliver them.

Discovering CenterWell Senior Primary Care

My search for a genuinely patient-centered, value-based care model eventually led me to the doors of CenterWell Senior Primary Care. At CenterWell, and its sister brand Conviva Care Center, the mission is to provide patient-centered, outcome-driven care, but the organization also takes great care of physicians by providing the autonomy, resources and support systems to achieve these goals. In my eyes, this was an immediate differentiation from other providers in the value-based care space, and I was ready to join the team.

In my current role, I work with a very manageable number of daily patient visits, allowing for longer appointment times and a more thorough, personalized approach to care. Now I have time to thoroughly prepare for their visit with my care team and spend more time getting to know my patient during the visit – which is my favorite part! CenterWell’s approach has been a game-changer for my patients and me, showing a significant reduction in emergency room visits and hospitalizations.

The dedicated care teams at CenterWell and Conviva include nurse practitioners, social workers, nurse care coordinators, behavioral health specialists and a clinical pharmacist. They provide invaluable support to me with patient care duties, documentation, administrative tasks and a lot more, which eases the burden of my day-to-day tasks and allows me to give my full attention to my patients. The assistance from our clinical pharmacists is extremely useful and practical, as they help manage patient prescriptions, medication inquiries, ensuring proper medication adherence with quality measures such as HEDIS and lower medication cost initiatives helping to drive down medical costs.  All of these tasks help to alleviate the workload on physicians. This collaborative approach is vital to the success of a value-based care model, allowing CenterWell and Conviva providers to address all social, financial, mental and clinical factors impacting our patients, from basic needs to chronic disease management.

Taking Care of the Patients, and Providers

Preventing physician burnout has always been a top priority at CenterWell. The clinical leadership team empowers me to provide high-quality care by supporting me with a robust care team, implementing innovative technologies to ease administrative burdens, and offering flexible scheduling that aligns with my personal commitments, especially in our time of need. For instance, when I had to take leave to care for my parents, the leadership team was very accommodating, allowing me to manage both my personal and professional responsibilities effectively. This, compounded with the autonomy I have over my schedule and appointment frequencies, ensures that both my needs and the needs of my patients are thoroughly met. Thus, I’m able to provide high quality care to my patients without feeling overwhelmed and stressed as so many providers feel today.

CenterWell also encourages the use of innovative technologies that benefit both patients and providers. Advanced Electronic Medical Record (EMR) systems and dictation software platforms streamline the documentation process, allowing more patient-facing time, which helps me develop a deeper relationship with my patients. This technological edge significantly reduces the administrative load on physicians, contributing to a more efficient and fulfilling work environment.

While I appreciate the personal benefits of working at an organization like CenterWell and Conviva, I am equally proud that the models and processes we implement consistently deliver results for patients. It has been a truly transformative experience. I feel so fortunate to have found an organization that not only benefits our patients, but also ensures providers are supported, empowered and fulfilled. For physicians seeking to make a meaningful impact in geriatric care while enjoying an enriching work environment, CenterWell and Conviva offer an ideal opportunity.
