Dr. Alex Foxman has had an impressive career as both a physician and successful entrepreneur, running three different types of practices — a virtual medical weight management program, a physical primary care medical practice and a mobile medical practice serving the frail elderly population with complex needs.

Being both a doctor and businessperson is a significant time commitment, but giving patients top-notch care that is not confined to the restrictions of corporate medicine is worth the additional investment.

Dr. Maryal Concepcion, host of the podcast My DPC Story, has also prioritized a holistic approach to patient health. She interviews direct primary care providers like Dr. Foxman, who focus on decisions made by patients and doctors, rather than by a health plan.

Unfortunately, that isn’t the case for many primary care providers.

“It used to be that most physicians were private practice physicians, but now less than 30% are in private practice, and it’s shrinking. That's because we’re really being pushed out by corporate medicine,” Dr. Foxman said. “Everyone is just a number sitting on a seat, and I never wanted my patients to feel that way.”

But to maintain highly-efficient medical practices independent of corporate backing, Dr. Foxman knew he wouldn't get very far without an equally efficient EHR ecosystem.

 

The Problem

For over a decade, Dr. Foxman used the same EHR as a nearby hospital’s outpatient clinics, assuming it would be an integrated and high-quality option. But almost immediately, it became painfully obvious that maintaining the server-based, bug-prone system would necessitate tremendous amount of maintenance and cost.

“It took eight server backups, a tech person to oversee it, and it wasn’t customizable. Every little tweak cost a huge amount of money,” he said. “I went to the office on the weekends to do backups.”

While the technology was burdensome, Dr. Foxman felt he had little choice.

As time went on, he knew the servers were becoming outdated, and it was only a matter of time before the security concerns potentially impacted personal health information. He decided to explore other EHR options.

 

The Solution

Because each clinic had its own unique workflows and requirements, the new EHR system had to be able to grow and adapt to each practice.

 

Mobility

DrChrono’s cloud-based system allows providers and staff to operate the mobile clinic, but it also saves staff time at the physical primary care office. Now, staff can take notes and send orders or information from iPads, laptops and even phones.

“I used to have to drag a laptop around with me. Now, I can just use a tablet,” he said. “There is a huge amount of mobility and independence that I never had before.”

 

Partner and physician networks

While DrChrono’s EHR experts are at customers’ disposal, Dr. Foxman has found their physician sharing and API partner network particularly helpful as well. Providers within their respective specialties can share their own customized workflows and templates, and DrChrono’s open API lends itself to a myriad of helpful applications and resources that can supplement one’s practice.

Between running her own direct primary care practice and interviewing doctors on My DPC Story, Dr. Concepcion has become increasingly confident that EHRs with open APIs are a must.

“If you have an open API, you are welcoming innovation and a progressive approach to the future,” she said. “You need to be able to have accessibility for your patients.”

 

Communication and task management

Patient-provider relationships do not start and end at the scheduled appointment times.

Patients at any of Dr. Foxman’s practices can easily log in to their patient portal to send and receive messages, receive results and follow-up education.

“I think patients are left out of their healthcare too often,” Dr. Foxman said. “For our patients, it’s the ease of communication, education and contact with staff that has been very important.”

Even giving directions to staff, such as drawing a patient’s blood, can be done through their centralized EHR system, which also allows for seamless task management.

 

Customizability

One of the most appealing aspects of DrChrono is the ability to tailor one’s own workflows and notes to your practice, without the need for in-depth technical expertise. Creating forms is intuitive and only requires a short amount of time.

Now, DrChrono sits at the heart of Dr. Foxman’s practices, which in turn allows for stronger patient relationships.

“DrChrono is used so differently in all these different aspects, and that’s what I love about it,” he said. “We can use it in the office space, we can use it in the mobile clinic, or we could use it for the virtual program.”
