AtlantiCare’s Primary Care practices are taking meaningful steps toward the “patient-centered medical home” model through a commitment to same- or next-day urgent appointments, electronic medical records and collaboration with an AtlantiCare Care Manager for complex patients. Additionally, the practices are supported by AtlantiCare Hospitalists, who care for patients admitted to our two medical centers.

The successful candidate will possess strong clinical, communication and consultation skills, as well as a patient-first focus. He or she will aspire to drive improvement in healthcare well into the future.
